Limestone Medical Center is committed to protecting our patients’ rights and privacy. All personal health information is kept strictly confidential and safeguarded in accordance with federal HIPAA Privacy and Security regulations. Patients, parents of minor patients, and authorized caregivers (including those with Healthcare Power of Attorney) have the right to review or obtain copies of medical records, or request that records be sent to another provider or facility.

Obtain Printed Copies of Your Medical Records
Need assistance obtaining a hard copy of your medical records or have questions about them?
Please contact the appropriate medical office for support. Refer to the flyer linked HERE for contact numbers associated with Limestone Medical Center.
To request a hard copy of your medical records, please download and complete the Release of Information form below. Once completed, you may submit the form to your physician’s office in person or contact the office to arrange an alternative method of submission.
- Photo Identification is required.
- Complete and sign the release of information authorization form.
