Community Health Needs Assessment
Limestone Medical Center (LMC) relies on the Community Health Needs Assessment to guide the organization’s mission-driven work to inspire healthier lives and stronger communities. Community priorities become part of LMC’s overall strategy through the practice of listening to the voices of our community, developing Community Health Improvement Plans informed by data, and ultimately seeking approval from our board of directors
The enactment of the Patient Protection and Affordable Care Act (PPACA) requires tax-exempt hospitals to conduct these assessments and implementation strategies every three years. We are required to make these documents available to the public. Section 501(r)(3) requirements include having an authorized body at the hospital facility adopt a documented CHNA that is available to the public and available for feedback.
