Position Summary
The Population Health Coordinator is responsible for ongoing patient engagement, care coordination, and removal of barriers to care for high-risk and chronic disease populations. This role ensures sustained participation in care plans, supports preventive services, and contributes to improved clinical outcomes and reduced avoidable utilization.
Working closely with the Transitional Care Registered Nurse, Nurse Practitioner/Physician Assistant, and Registered Dietician, the Population Health Coordinator plays a key role in maintaining continuity of care and improving population health performance metrics.
Required Qualifications
- High school diploma or equivalent required; associate or bachelor’s degree in healthcare-related field preferred
- Experience in healthcare, care coordination, case management, or patient services
- Strong communication and organizational skills
Preferred Qualifications
Experience with:
- Population health or quality programs
- Chronic disease management
- Rural or underserved populations
- Familiarity with EHR systems (e.g., CPSI/TruBridge)
- Skills and Competencies
