Nurse Practitioner – Flexible House Call Program
Are you an experienced FNP-C looking for a flexible way to earn supplemental income while making a meaningful impact in your community?
Limestone Medical Center House Call Program is looking for a part-time Independent Contractor to provide in-home visits to geriatric patients. Work under the supervision of a highly experienced, board-certified physician in Internal and Geriatric Medicine. This role offers autonomy, flexibility, and the chance to form genuine connections with patients in their homes.
What You’ll Do:
- Conduct a minimum of 80 patient visits per month, with opportunity for more!
- Deliver compassionate care to elderly patients in their home setting
- Perform routine health assessments, chronic care management, and preventive services
- Collaborate with an experienced physician and supportive care team
- Maintain records using your own device
- Join monthly Zoom check-ins with the team
What We’re Looking For:
- Texas Licensed Nurse Practitioner (FNP-C) or Physician Assistant
- At least 3 years’ clinical experience (geriatrics/home care preferred)
- DEA license, own malpractice insurance, computer, and transportation
- Excellent communication skills and comfortable working independently in patients’ home
- Passion for serving older adults and dedicated to their care
