JOB TITLE: House Call Nurse Practitioner
DEPARTMENT: House Call Program
JOB SUMMARY: Provides safe and effective care to ambulatory patients under the supervision of a collaborating physician. Utilizes independent judgment in the assessment, diagnosis, treatment planning, and provision of ongoing care to ambulatory patients that is in accordance with nurse practitioner licensure regulations for Texas and the Prescriptive Authority Agreement.
Program Overview: The Limestone Medical Center (LMC ) House Call program is specifically tailored to support elderly patients in their desire to age comfortably at home, aiming to minimize hospital readmissions and overall healthcare costs. This initiative is driven by an interdisciplinary team that delivers comprehensive primary care directly to patients in their homes or assisted living facilities. Typically serving patients aged 65 and above who face challenges in attending traditional medical appointments, the program primarily assists individuals managing multiple chronic conditions, coping with changes in functional ability, handling polypharmacy, and dealing with frailty or debility.
JOB QUALIFICATIONS:
- A. Education: For Nurse Practitioners: Bachelor’s Degree in Nursing and Master’s Degree in Advance Clinical Practice or graduate of a post-masters certificate program for nurse practitioners, OR certificate from an education program that is accredited by the Council of Nurse Ansethesia Educational Programs/Schools, American College of Nurse Midwives, American Association of Colleges of Nursing of the National League for Nursing; For Physician Assistants: Graduate of an accredited Physician Assistant program.
- Personal Job-Related Skills: Ensures ongoing growth and development of clinical skills and knowledge by assuming responsibility for lifelong learning. Provides direction and guidance to others regarding practice, also serves as a resource and mentor. Demonstrates leadership skills through participation in program development.
- Licensure, Registry or Certification: State License, Valid DEA, Graduate from an accredited program.
Certification: Basic Life Support upon star; Advance Cardiovascular Life Support (for Critical Care) upon start; Pediatric Advanced Life Support (for Pediatrics) upon start.
Licensure: Licensed Physician Assistant or Nurse Practitioner; If Nurse Practitioner: RN Compact License (depending on State of primary residence) AND a license (or eligibility for) as a nurse practitioner or physician assistant in the Commonwealth of Virginia AND a “Practice Agreement” signed by sponsoring/supervising physician(s), And a written “Scope of Practice/Protocol” signed by the supervising physician, AND “Drug Enforcement Administration License” (if prescribing controlled substances.)
Experience:
1. Prior Work Experience: Minimum 3 years as Nurse Practitioner
